REIMBURSEMENT OF COACH COURSE EXPENSES
Please complete the online form (available here) and attach a scanned copy of the receipt of payment for the course(s) that were taken.  This will be submitted to the Registration Director for review and approval. Once approved, a cheque will be forwarded to the address you provided in the form submission within 6 to 8 weeks of approval.  Any questions or concerns can be directed to: [email protected].

Please be sure that your scanned receipt is legible to avoid delays in reimbursement AND/OR rejection of your submission.



REIMBURSEMENT OF POLICE CLEARANCES
Please complete the online form (available here) and attach a scanned copy of your Vulnerable Sector Check and a copy of the receipt of payment.  This will be submitted to the Mission & Ethics Director. A cheque will be forwarded to the address you provided when LMHA receives both of these documents within 6 to 8 weeks of approval.  Any questions or concerns can be directed to: [email protected].

POLICE DECLARATION
For those that are in Year 2 or Year 3 of a valid Police Clearance, you are required to submit a Police Declaration.  The police declaration can be found here.
