REIMBURSEMENT OF COACH COURSE EXPENSES
Please complete the online form (available here) and attach a scanned copy of the receipt of payment for the course(s) that were taken. This will be submitted to the Registration Director for review and approval. Once approved, a cheque will be forwarded to the address you provided in the form submission within 6 to 8 weeks of approval. Any questions or concerns can be directed to: [email protected].

Please be sure that your scanned receipt is legible to avoid delays in reimbursement AND/OR rejection of your submission.

NSTRUCTIONS FOR SUBMITTING POLICE CLEARANCES TO OHF

The Police Record Check (PCR) needs to be uploaded by each person through the Ontario Hockey Federation Vulnerable Sector Checks screening process. The link to do so is below:

Before you start, you will need a PDF of the Police Record Check and an HCR number. The HCR number is obtained through Spordle (https://myaccount.spordle.com/login). If you have not created an HCR number, you can log into spordle using the same account you use when registering your kids for hockey. From the Spordle dashboard, then select Register Now and search for Lasalle MHA and register as COACHES, BENCH STAFF, VOLUNTEERS, ON-ICE HELPERS for the current season. This will give you an HRC number. It is the long number that will appear under your name once you have registered. Once you have the number you can proceed with the screening process above. You will know that your screening is approved by going back onto spordle and seeing the green “valid background check” badge below your name.

 

One thing to keep in mind when doing the screening process is that it has a separate link on the form that you have to click to actually upload the PRC. You have to click the link and upload the PRC before you submit the form. It will allow you to submit the form without first uploading the PRC, but there is then no way to upload it, you will get an email saying that your screen cannot be completed, and you will have to start a new submission and fill in the all the information again. I did this the first time with mine, so be careful and read the form carefully before submitting it.

POLICE DECLARATION

For those that are in Year 2 or Year 3 of a valid Police Clearance, you are required to submit a Police Declaration.
