Use this form to register and pay for a player clinic that is being offered by LMHA. At this time only full duration sign up is being offered. Dates: Sept 21, 28 Oct 5, 12, 19, Nov 2, 9, 16, 23, 30 Dec 14, 21, 28

On Ice Helper Request

NOTE: Any helpers 14 and under MUST wear full equipment on the ice.

Payment for these clinics is required in order for any player to participate.  Only those who have paid will be eligible to participate.
