ALL VOLUNTEERS/POLICE CLEARANCES - VULNERABLE SECTOR CHECK OR DECLARATION REQUIRED
UPDATE FOR 2020/2021 SEASON 

All coaches, trainers, managers, on-ice helpers and volunteers require a valid Level 3 Vulnerable Sector Check dated 2018 or newer
.

The Vulnerable Sector Check can be obtained online from the police service where you reside and you will be reimbursed for the cost by LMHA.  See "Course/Police Clearance Reimbursement Process" under "COACHES" on the LMHA webpage or click here.  

The form for submitting the police clearance and the request for reimbursement can be found here as well as under Coaches, Coach Police Clearance/Declaration Forms. 

Volunteers who have a criminal record check from 2018 or 2019 are required to complete the LMHA Declaration located here as well as under Coaches, Coach Police Clearance/Declaration Forms.  If you are advised by the police service that you require a letter from LMHA, please email the Mission and Ethics Director at [email protected].  You will need to provide your full name, date of birth and complete address.
